Wie op internet naar borstcorrectie-websites surft, zal vast al gemerkt hebben dat de overgrote meerderheid (voornamelijk Amerikaanse sites) het heeft over watergevulde ronde en vaak gladde implantaten onder de borstspier geplaatst. Als je in Nederland en België een plastisch chirurg raadpleegt, raadt die je vaak ruwe druppelvormige siliconengevulde implantaten boven de borstspier aan. Precies het tegenovergestelde dus. Hoe komt dit nu? Kennen de Amerikanen niets van implantaten of hebben de Europeanen het bij het verkeerde eind? Genoeg aanleiding voor mij om deze controverse te proberen op te helderen in deze column.
Om het fenomeen borstvergroting in de VS te kunnen begrijpen, moet je eerst iets meer weten over het medisch rechtssysteem in de VS. In de VS is het toegestaan dat advocaten werken volgens het “no cure no pay” principe. Dit betekent dat een advocaat met een client kan afspreken dat als hij de rechtzaak verliest, er niets aan de advocaat hoeft betaald te worden. Dit is in Europa veboden. Dus in theorie mag het ereloon van een advocaat hier niet gelinkt worden aan de uitslag van het proces. Dit betekent dat als je ook maar een kleine kans ziet op schadevergoeding in de VS je gewoon naar een advocaat stapt om een rechtszaak in te spannen. Als je het proces verliest, kost het je in principe toch niets.
Vandaar dat in de VS veel patienten vaak “groot geld” zien in het voeren van een proces tegen hun arts. Uiteraard is het succes van een dergelijke mentaliteit van korte duur en kreeg de patient in de VS het deksel op zijn neus. Artsen doen enkel nog dingen waarvan ze bijna zeker zijn dat er geen proces kan gevoerd worden. Niet de beste behandeling maar de behandeling met het laagste “procesrisico” wordt steevast gekozen. Daarbij komt nog dat de artsen zich op een soms ridicule manier proberen in te dekken. Zo bezocht ik enkele jaren geleden een ziekenhuis in Washington. Aan de ingang van de spoedgevallen zaten 24 uur op 24 twee advocaten van het ziekenhuis. Wie een been breekt moet eerst langs de advocaat om te tekenen dat hij op de hoogte is dat een gips afklemming van de bloedvaten kan veroorzaken en dat de kans bestaat dat je door de gips je been verliest!!
Begin 1990 zagen een aantal patienten groot geld in hun siliconenprotheses. Ze beweerden namelijk dat je er kanker, gewrichtsaandoeningen, huidaandoeningen,… van kreeg. Nu is het zeer moeilijk om te bewijzen dat zonder de protheses deze aandoeningen ook zouden zijn opgetreden en dat de associatie louter toeval was. Een aantal dames kreeg dan ook een fikse vergoeding toegewezen. Gezien de vele claims die daarop volgden, is er dan uiteindelijk een moratorium voor siliconengevulde protheses afgekondigd. Siliconengevulde protheses zijn dan ook sinds ongeveer 8 jaar verboden in de VS. Vandaar dat alle Amerikaanse sites enkel over watergevulde protheses spreken. Ondertussen werden gigantische studies uitgevoerd op siliconen. Siliconen is heden dan ook ongeveer het meest onderzochte product voor geneeskundig gebruik. Geen enkele studie kon een verband met kanker of andere aandoeningen aantonen. De medische wereld is er nu dan ook van overtuigd dat siliconen volledig veilig zijn.
Heden lopen er massa’s claims binnen in de VS over leeglopen van zout gevulde protheses. Er gaan dan ook stemmen op in de VS om over te schakelen op cohesieve gel gevulde protheses i.p.v. zout gevulde. Zo zie je maar dat de keuze van de prothese in de VS niet altijd op medische basis maar vaak op basis van juridische overwegingen wordt bepaald.
Waarom zout gevulde implantaten meestal rond, glad en onder de spier geplaatst worden, zal in een volgende artikeltje belicht worden. De voor-en nadelen van siliconen versus watergevulde protheses zullen eveneens in een latere column worden toegelicht.

Siliconenborstprothesen

Druppelvormige (anatomische) siliconenprothesen

De vulling van deze borstprothese bestaat uit hoog-cohesieve siliconengel. Alle fysieke eigenschappen zijn getest op lekkage, cohesie (samenhang) en compressie (samendruk). Zoutwater-gevulde borstprothesen
Voordat er siliconenprothesen beschikbaar kwamen, waren borstprotheses gevuld met zout water. Dat is nog steeds een alternatief. Alhoewel nog nooit bewezen is dat siliconen schadelijk zijn voor het lichaam, weet u bij zout water zeker wat er gebeurt in geval van lekkage. De prothese-inhoud loopt leeg en het zoute water wordt door het lichaam geresorbeerd. Zout water heeft echter niet dezelfde souplesse als siliconen en is daardoor minder geschikt voor magere vrouwen. Is een vrouw licht gebouwd, dan bevindt zich weinig weefsel tussen borstklier en de prothese, waardoor de hardheid van de prothese beter voelbaar is. Veelal worden protheses pas na het inbrengen met zoutwater gevuld. De kans op lekkage (via de klep waarlangs het zoute water er ook in is gebracht) is bij deze prothesen aanzienlijk groter. Deze prothesen zijn niet onze eerste keus in verband met bovengenoemde nadelen.
